How to get NSO Birth Certificate, Marriage Certificate, Death Certificate & CENOMAR?






Nowadays, it is so easy to get a Copy of your NSO Birth Certificate at NSO Head Office in East Avenue. I can that some of our Government agencies really strived in changing when it comes in providing services to our fellow constituents. In addition, they really deserved acknowledgement for their good service.



I arrived at the NSO East Avenue around 4:00 P.M. and believing that I will be coming back tomorrow for my NSO Birth Certificate but to my surprise when I saw my receipt that I can get it on 5:30 P.M. on the same day! I was glad and relieved since I lived in Rizal I don’t need to travel again.


Here’s the step;



Stage 1 

Get white application form from the counter and Application form are color coded (White - Birth, Pink - Marriage, Yellow - Death, Green - Certificate of No Marriage Record) so you are not be confused. Fill the required fields in the form.


Birth Certificate: 

  • Complete name of the child (first, middle, last) 
  • Complete name of the father 
  • Complete maiden name of the mother 
  • Date of birth (month, day, year) 
  • Place of birth (city/municipality, province) 
  • Whether or not registered late. (If registered late, please state the year when it was registered. 
  • Complete name and address of the requesting party 
  • Relationship to the child 
  • Number of copies needed 
  • Purpose of the certification 

Marriage Certificate:

  • Complete name of the husband 
  • Complete name of the wife 
  • Date of marriage 
  • Place of marriage 
  • Complete name and address of the requesting party 
  • Number of copies needed 
  • Purpose of the certification 

Death Certificate:

  • Complete name of the deceased person 
  • Date of death 
  • Place of death 
  • Place of marriage 
  • Complete name and address of the requesting party 
  • Number of copies needed 
  • Purpose of the certification 

CENOMAR (Certificate of No Marriage)

  • Complete name of the person 
  • Complete name of the father 
  • Complete maiden name of the mother 
  • Date of birth 
  • Place of birth 
  • Complete name and address of the requesting party 
  • Number of copies needed 
  • Purpose of the certification 


Stage 2

Proceed to covered court for the Screening and Cashier Area and take token (number) from the security guard way in. No worries since there are 12 counter so it will take a 20 – 30 minutes maximum. You have to present original one (1) valid id although it is your own document that you are requesting they will require you to present it.
  • Valid IDs; 
  • Driver’s License 
  • PRC License 
  • Voter’s ID 
  • Postal ID 
  • Passport 
  • Seaman’s Book 
  • SSS 


Stage 3

Screening and Payment; once your number shows in the monitor stand up and show your token number to the guard and he will tell you which counter you have to go to. Pay only at the cashiers window and after payment check the details on your receipt (name and type of certificate requested). Notify the cashier officer if something wrong in your details.


Fees;

P140.00 (Birth, Marriage, Death Certificate)
P195.00 (CENOMAR - Certificate of No Marriage Record)

Their cut-off timings are until 5:00 pm. In my experience, I came there around 4:00 pm and I finish paying at 4:30 pm and luckily I collected my Birth certificate at 5:30 pm on the same day.



Stage 4

Check your receipt because the officer writes down the information on your receipt the details of time and date you can collect your NSO Birth, Marriage, Death Certificate or CENOMAR and which window you have to claim it..



Stage 5 

Releasing of certificates, this is located behind the screening and payment area. There are 9 windows for releasing. If your schedule of release is on the same day just wait until the guard announce the "time of release". Otherwise, you can not enter the releasing area, waiting until the guard on duty will announce the "time of releasing" (you can check in your receipt). Then one by one thw guard will call the time and the window number to fall in line.

If you missed your timing, don't worry you can still collect just inform the guard that you missed you timings. Actually, I was surprise because the guards are very kind. I really salute them kahit na mainit, they don't bother just to serve the people.

Another thing, they will disposed all unclaimed documents within 30 days from the date of release. So, make sure you claim your documents before 30 days of the release date.

Plus, do not forget to bring a valid ID.If you are applying or claiming for another person, you need to bring authorization letter and a valid ID from the person named in the document.
